Streszczenie: W pracy przedstawiono procedury opracowane na podstawie dwudziestoletniego doświadczenia w replantacjach kończyn u dzieci i dorosłych. Omówiono m.in.: wskazania i przeciwwskazania do tego zabiegu, warunki transportu, system pracy bloku operacyjnego, wybrane zagadnienia śródoperacyjne oraz pooperacyjne postępowanie farmakologiczne. Przeważająca liczba udanych replantacji w materiale klinicznym, na którym opierają się procedury, została wykonana pomimo ogólnie przyjętych przeciwwskazań do tego zabiegu. Nadrzędnym celem replantacji nie było uzyskanie przeżycia replantowanej części kończyny, tylko poprawa jej funkcji w stosunku do stanu po amputacji. Oprócz wypracowanego modelu postępowania, w przedstawionych procedurach zawarto przesłanie, że – pod warunkiem zapewnienia bezpieczeństwa w trakcie i po operacji – jedynym przeciwwskazaniem do replantacji jest brak możliwości jej wykonania.

Abstract: In this paper was presented the procedures developed and based on 20 years of experience in limbs replantation in children and adults. The author presents, among others, indications and contraindications, transport conditions, system of work in the operating theater, selected issues intraoperative and postoperative pharmacotherapy. A prevailing number of successful replantation in clinical material on which are based the procedures, were performed in the generally accepted counter indications for the replantation. The superior aim of replantation surgery is not only survival of the replanted part, but the functional outcome. Besides to the description of the model treatment that is called “The Krakow Procedures in Replantations”, the model includes a message that if the procedures are provided safety during surgery and after the surgery, the only contraindication for limb replantation is the inability of replantation.
