The indicators of erythrocytes energy supply and antioxidant status in patients suffering from severe burn shock

Abstract: In the presented study, the dynamics of energy supply and antioxidant energy status of erythrocytes was evaluated in adult patients with severe thermal burn shock. The concentration of lactate was found to be increased over two times in all patients suffering from severe burn shock. It indicates an oxygen deficit in the tissues of heavily burnt patients. Moreover, the development of severe hypoxia, energy deficit and the significant tension of antioxidant defense of erythrocytes was noticed during the first twenty-four hours after a burn. Our results indicate that ratio of energy supply and antioxidant status of erythrocyte is dependent on the severity of the injury, what suggests the necessity of the rapid application of anti-hypoxic and antioxidant measures. The activity of adenosine deaminase in erythrocytes may be considered the indirect marker of hypoxia level. Despite the treatment applied, the significant drop of recovered glutathione (1.5×) was observed. This phenomenon was related to the exhaustion of the patients’ antioxidant system. Only patients with the evaluated superoxide dismutase level survived the burn shock phase, what confirms the key role of aforementioned enzyme in antioxidant defense in burned patients.

Streszczenie: W pracy oceniono dynamikę dostarczania energii oraz antyoksydacyjny status erytrocytów u dorosłych pacjentów w szoku termicznym. Zaobserwowano u nich ponad dwukrotnie podwyższone stężenie mleczanu w porównaniu z grupą kontrolną, co wskazuje na deficyt tlenowy w ciężko poparzonych tkankach. W pierwszych 24 godzinach od urazu zaobserwowano także rozwój ciężkiej hipoksji oraz deficyt energetyczny w erytrocytach. Uzyskane wyniki wskazują na zależność między poziomem energii oraz statusem antyoksydacyjnym erytrocytów a stopniem oparzenia. Wskazuje to na konieczność szybkiego zastosowania środków przeciwutleniających i działających przeciwko niedotlenieniu. Aktywność erytrocytowej deaminazy adenozynowej może być uważana za pośredni marker stopnia niedotlenienia. Mimo zastosowanych opcji terapeutycznych, zaobserwowano znaczący spadek glutationu (1,5×). Zjawisko to powiązano z osłabieniem systemu antyoksydacyjnego pacjentów. Przeżyli jedynie chorzy, u których doszło do aktywacji i nadprodukcji dysmutazy nadtlenkowej, co potwierdza kluczową rolę tego enzymu w antyoksydacyjnej ochronie tkanek poparzonych.
