Streszczenie: Wstęp Zakażenia skóry i tkanki podskórnej to zróżnicowana jednostka chorobowa o szerokim spektrum objawów, różnym przebiegu i rokowaniu. Wnioski Cefiderokol i ceftazydym z awibaktamem wykazują wysoką aktywność wobec P. aeruginosa izolowanych z zakażeń skóry i tkanki podskórnej, przewyższającą aktywność meropenemu i fosfomycyny.

Abstract: Introduction Skin and soft tissue infections (SSTIs) constitute a heterogeneous group of diseases characterized by a wide spectrum of clinical manifestations, variable clinical course, and diverse prognostic outcomes. The risk of complications is strongly influenced by patient age and the presence of underlying comorbidities. One of the major etiological agents is Pseudomonas aeruginosa, a non-fermenting Gram-negative bacillus. This pathogen possesses numerous virulence factors, including a unique outer membrane structure, the production of proteases and pigments, and the ability to form biofilms. Aim The aim of this article is to present the role of Pseudomonas aeruginosa as an etiological agent of skin and soft tissue infections and to assess the activity of two novel therapeutic options – cefiderocol and ceftazidime–avibactam – against multidrug-resistant strains. P. aeruginosa exhibits intrinsic resistance to multiple antimicrobial agents and readily acquires additional resistance mechanisms, posing significant therapeutic challenges. The increasing prevalence of multidrug-resistant (MDR) P. aeruginosa strains has further limited available treatment options, underscoring the need for novel antimicrobial therapies. Among the recently introduced agents, cefiderocol, a novel siderophore cephalosporin, has demonstrated excellent in vitro activity against P. aeruginosa, including MDR isolates. Similarly, ceftazidime–avibactam has shown high antimicrobial activity against this pathogen and represents an important therapeutic option for the treatment of infections caused by resistant strains. Conclusions Both cefiderocol and ceftazidime–avibactam demonstrate high activity against P. aeruginosa isolated from skin and soft tissue infections, exceeding that of meropenem and fosfomycin. Cefiderocol retains activity against metallo-β-lactamase-producing strains, against which β-lactam/β-lactamase inhibitor combinations remain ineffective. Both agents represent valuable options for targeted therapy; preserving their efficacy requires susceptibility-guided use and adherence to antimicrobial stewardship principles.
