Leczenie obejmowało wieloetapowe postępowanie chirurgiczne z użyciem stabilizacji drutami Kirschnera, regularną pielęgnację rany z wykorzystaniem nowoczesnych opatrunków specjalistycznych, preparatów antyseptycznych, opatrunków zawierających srebro nanokrystaliczne, hydrożeli oraz żelu z hialuronianem sodu i aminokwasami.

Abstract: Torsional crush injuries of the upper extremity constitute a significant therapeutic challenge due to the high risk of complications, including the loss of function or complete amputation of the affected segment. The aim of this study was to present the management of a complex post-traumatic wound following subtotal amputation of the right index finger in a 40-year-old firefighter and to evaluate the effectiveness of the local and systemic interventions applied. The patient sustained an open comminuted intra-articular fracture of the proximal phalanx of the right index finger (Gustilo-Anderson type III C) as a result of a torsional crush injury. The treatment involved a multi-stage surgical approach with Kirschner wire fixation, regular wound care using advanced dressings, antiseptic agents, nanocrystalline silver dressings, hydrogels, and a gel containing sodium hyaluronate and amino acids. Adjunctive therapy included pharmacological treatment, particularly pentoxifylline, as well as high-protein nutritional supplementation enriched with arginine. Despite the high risk of tissue devitalization, necrosis, and finger amputation, gradual improvement of the local wound condition was achieved. A particularly beneficial effect on wound healing dynamics was observed following the introduction of a sodium hyaluronate and amino acid gel formulation. The treatment also required a subsequent surgical intervention due to pseudoarthrosis and bone loss. Ultimately, complete wound healing was achieved 109 days post-injury, and the affected finger was successfully preserved. This case demonstrates the importance of a comprehensive, multidisciplinary strategy involving surgical management, local wound care, targeted pharmacotherapy, and nutritional support. Moreover, early referral to a specialized upper-limb trauma center was pivotal in achieving a favorable outcome, preventing amputation, and facilitating the patient’s return to professional activity.
