Streszczenie:

Zgorzel Fourniera to rzadkie martwicze zapalenie powięzi krocza o szybkim przebiegu, obarczone nadal wysoką śmiertelnością. Szybka interwencja chirurgiczna połączona z antybiotykoterapią pozostają kanonem w leczeniu ostrej fazy choroby. Istnieje natomiast wiele metod zaopatrzenia ubytku tkanek miękkich po resekcji martwicy. Celem pracy jest przedstawienie nietypowego przypadku zgorzeli Fourniera u pacjenta z paraplegią. Nowoczesne metody leczenia chorego okazały się nieskuteczne. W związku z tym zastosowano: opatrunki z pastą srebrową, wspomagające leczenie w komorze hiperbarycznej i etapowe chirurgiczne zamykanie rany. Mimo problemów wynikających z objęcia procesem chorobowym okolicy odbytniczej i odsłonięcia jąder u chorego po uszkodzeniu rdzenia kręgowego, ostatecznie rana wygoiła się bez ubytku funkcji.

Abstract:

Fournier’s gangrene is a rare necrotizing fasciitis of the perineum, rapidly progressing, still burdened with high mortality. Urgent surgical intervention combined with antibiotics remains the standard of care in the acute phase. There are, however, many methods of wound closure after debridement. The aim of this paper is to present an unusual case of Fournier’s gangrene in a paraplegic patient. When modern methods of wound treatment failed, we used silver alginate wound paste, combined with hyperbaric oxygen therapy and staged surgical closure. Despite the wound involving the anal region and testes in a patient after spinal cord injury, we managed to close the wound without loss of function.
