Streszczenie: Wstęp W niniejszej pracy przedstawiono metodę wykorzystania miejscowych płatów perforatorowych typu „keystone” w rekonstrukcji ran kończyn. Opisana technika rekonstrukcyjna spełnia takie wymagania jak: tworzenie odpowiedniego pokrycia tkankowego głębokich struktur anatomicznych z ich rewaskularyzacją oraz zapewnienie akceptowalnego wyglądu i dobrej jakości życia pacjenta. Kluczem metody „keystone” jest koncepcja аngiosomów i angiotomów. W pracy przedstawiono projektowanie i technikę zaznaczania płata oraz przytoczono przykłady wykorzystania niniejszej metody zakończone sukcesem. Materiały i metody W okresie od 2011 do 2015 roku w Centrum Leczenia Oparzeń i Chirurgii Plastycznej w Dniepropietrowsku w celu zamknięcia głębokich i rozległych ubytków powłok kończyn zoperowano 33 pacjentów (30 mężczyzn i 3 kobiety w wieku od 20 do 64 lat). W grupie tych chorych wykonano rekonstrukcję utraconej skory na kończynach z wykorzystaniem plastyki miejscowymi płatami „keystone” (łącznie 47 przypadków). Wyniki U wszystkich pacjentów rozległe rany zostały całkowicie zamknięte jednoetapowo; chorzy zostali wypisani ze szpitala w dobrym stanie. Wszystkie płaty wgoiły się, a ubytki tkanek zostały wyeliminowane w jednym etapie. Wnioski Metoda plastyki lokalnymi perforatorowymi płatami „keystone” pozwala zwiększyć skuteczność zabiegow rekonstrukcyjnych w przypadku zamykania głębokich i rozległych ran kończyn. Dzięki przedstawionej technice w większości przypadków możliwe jest pierwotne jednoetapowe zamknięcie szerokiego ubytku, bez konieczności mobilizacji osiowego naczynia perforatorowego w dolnej części płata. Skutkuje to minimalną ilością powikłań pooperacyjnych i brakiem wtórnych defektów, charakterystycznych dla pól dawczych w przypadku wyboru alternatywnych metod.

Abstract: Introduction The method, which is described in the work, corresponds to the following requirements: the creations of an adequate cover of deep anatomical structures with their revascularization, restore an acceptable appearance and quality of life. The way to its implementation is the concept of angiosom and angiotom. The paper describes a design and dissection technique of island flap, provides the examples of the successful application of the method. Materials and methods During the period from 2011 to 2015 in the clinic were operated 33 patients, 30 men and 3 women, aged from 20 to 64 years, with plastic closure of deep and extensive wound defects of the extremities. Local keystone flaps were performed in 47 cases with purpose of reconstruction of lost skin of the extremities. Results In all cases, extensive wound defects were closed completely in one stage and the patients discharged with recovery. All flaps survived and wound defects were eliminated in one step. Conclusions Method of local perforator keystone flap improves the efficiency of reconstructive plastic surgery of the deep and extensive wound defects of extremities. This method in most cases allows primary closing of extensive defect in one step, without mobilizing the axial perforating vessels as in the pedicle flap. This is reflected in a minimal number of postoperative complications and the lack of secondary defects typical for donor sites when choosing alternative methods.
